The documents provided on this website are in PDF format. They are accessible by clicking on the links provided on the individual site pages. Some of the documents are for informational purposes only. However, some are forms that can be filled out and submitted for various reasons. Below are some helpful tips and information about viewing, saving, and printing the PDF documents on this site.

TO VIEW, PRINT, SAVE AN INFORMATIONAL DOCUMENT:
Click on the document’s link to view the document in a new browser window.
In some cases, Adobe Reader may have to be installed on your computer.

Get Adobe Reader Button 

Print the document by clicking on “File > Print” in the browser’s menu bar.
Save the document to your computer by clicking on “File > Save As…” in the browser’s menu bar.

TO VIEW, FILL OUT, PRINT, SAVE A FORM/APPLICATION:

  1. Click on the document’s link to view the document in a new browser window.
  2. Using the PRINT BUTTON provided on the form, print the blank form and fill it out manually. Then deliver in person or by USPS, or
  3. Click in the grey fields and type your information and check any boxes that apply, then using the PRINT BUTTON provided on the form, print the form and deliver in person or by USPS, or
  4. Click in the grey fields and type your information and check any boxes that apply, then using the SAVE AS BUTTON provided on the form, save the completed form to your computer, then send the saved form via e-mail attachment.
  5. Use the RESET FORM BUTTON to clear all information entered into the form. This clears ALL information you have entered and CAN’T BE UNDONE!

NOTES:

  • Blank forms can always be saved to your computer using the SAVE AS BUTTON for later use.
  • If you are saving a completed form to your computer, you may want to name that particular instance of the form a new name so that you can identify it quickly and avoid overwriting previously saved versions.
